HOLIDAY REQUEST FORM

In line with company policy relating to holiday leave, all employees must submit a formal request.

A separate request must be submitted for each block of leave.

All requests are considered based on existing leave requests received from all employees, week commitments and minimum staffing levels.

TWO WEEKS ADVANCE NOTICE REQUIRED ON ALL HOLIDAY REQUESTS.
